PREFERRED CUSTOMER PROGRAM REGULATION
HOSPITAL CLÍNICA BÍBLICA
1. General Provisions
1.1.This preferred customer program belongs to the Costa Rican Medical Services Association, hereinafter referred to as ASEMECO, owner of the "Hospital Clínica Bíblica" trademark.
1.2. Users of Hospital Clínica Bíblica that comply with the requirements defined in the present document may participate in this program. ASEMECO reserves the right to include other beneficiaries at its discretion.
1.3. Those participating in this program will understand that they accept and comply with the following conditions and limitations and with any future modification to this regulation.
1.4. ASEMECO reserves the right to modify or cancel regulations, standards, privileges, benefits and awards, partially or totally, as deemed necessary.
1.5. It is imperative that the cardholder strictly complies with the special or preferential treatment conditions that each card establishes for the members of the Preferred Customer program.
1.6. ASEMECO reserves the right to modify discounts of special offers in the Preferred Customer program.
1.7. The Preferred Customer program is a customized program. Mail and advertising material related to the program, will be sent to the address indicated in the subscription document of each member or will be communicated through the telemarketing system, email, postal service or any other communication method accepted by the beneficiary.
1.8. The cardholder will be responsible for notifying HOSPITAL CLÍNICA BÍBLICA of any change in address, PO Box, email or telephone number to guarantee an adequate flow of information between parties.
2. Definitions
2.1.ASEMECO: The name of Costar Rican Medical Services Association in the present regulation.
2.2. Program: Preferred Customer Program; the program in which frequent users of Hospital Clínica Bíblica, complying with the here stated requirements, may enjoy additional benefits.
2.3. Card: The card issued by ASEMECO to customers that are part of the Hospital Clínica Bíblica's Preferred Customer Program. They are identified by two colors, blue (Classic card) and gold (Gold card).
2.4. Cardholder: owner of only one of the three types of Preferred Customer cards that is a member of the program and enjoys its benefits.
2.5. Dose: Benefit for cardholders of the Preferred Customer Program for using in outpatient and inpatient services at Hospital Clínica Bíblica.
2.6. Points: Sum of doses earned by using Hospital Clínica Bíblica's services during a specified period.
2.7. Promotions: Activities with a specified term that Hospital Clínica Bíblica promotes, which grant doses or other additional benefits.
2.8. Benefits: The sum of doses that cardholders accumulate while using services or goods that ASEMECO provides through Hospital Clínica Bíblica, and are awarded through qualifying discounts according to the type of card.
3. Term
3.1. The term for this program is three months, effective on October first, two thousand eleven.
3.2. The information presented herein will be current as long as there are no new regulations that revoke it.
4. Image Rights
4.1. While using the benefits of the Preferred Customer program, the cardholder authorizes Hospital Clínica Bíblica to distribute, if deemed convenient, your name and photographs within the program's advertising, and the cardholder waives the right to claim image rights there forth.
5. Doses and Points Modification
5.1. ASEMECO reserves the right to correct, eliminate or discount cardholder accounts, purchases or doses that have been mistakenly granted.
5.2. ASEMECO reserves the right to grant purchases or supplementary doses arising from specific promotional campaigns.
6. Membership
6.1. In order to subscribe to the Preferred Customer Program, you must purchase a minimum of $200.00 (two hundred United States Dollars).
6.2. The following spending amounts will apply in granting membership cards:
a. From $200.00 (two hundred dollars) to $500.00 (five hundred dollars): Classic Preferred Card.
b. From $501.00 (five hundred one dollars) on: Gold Preferred Card.
c. From $1001.00 (one thousand one dollars) on: Platinum Preferred Card.
6.3. Memberships will be valid for three months and will be automatically renewed unless ASEMECO deems otherwise. The cardholder may upgrade from one card to another when he or she reaches
the minimum purchase level required for the new card. This amount will not be cumulative. It is the annual purchase average that the cardholder must reach. Upon acquiring the benefits of the new card, you will be waiving those of the previous card but keeping the accumulated doses to date.
6.4. If the customer reaches the minimum purchase level ($200) to obtain a card and to remain active within one of the categories, the customer acquires the membership of the Preferred Customer Program during three months from the date the membership is accepted. ASEMECO has a total of eight days after being requested to communicate the acceptance of your membership.
6.5. The participation of any employee of the following companies is excluded from the Preferred Customer Program: ASEMECO, ANVECO, S.A., SITERE. Likewise, professionals and companies that provide services within or for any of the above mentioned companies are excluded from participating in the program, unless written authorization from ASEMECO is provided.
6.6. ASEMECO reserves the right to check the monthly purchase volumes of members to guarantee the amounts that justify and extend the membership in the Program with the regulation.
6.7. Any membership may be suspended from the Program until any discrepancy or anomaly detected has been solved satisfactorily.
6.8. ASEMECO reserves the right to terminate any membership that, according to its own judgment, has committed fraud, abuse or violation of any of the natural purchase processes, benefit usage, award usage or any other aspect of the program's regulations. Upon being disqualified, cardholders will lose all their membership rights. All of the above without harm or waiver by ASEMECO of legal and out of court actions filed, according to the wrongdoing.
6.9. In case the cardholder terminates the contract, he or she will automatically lose rights on their purchases or accumulated dross, which will not be transferable to another cardholder. Terminated the contract is understood when the cardholder submits a written request to unsubscribe from the program or when in a period greater than three months, the cardholder does not use the Preferred Customer card.
7. The Card
7.1. Every new program member will receive a Preferred Customer identification card. In case of loss or damage, the customer must pay a $3 replacement fee, which must be done two working days after the request.
7.2. Discounts granted with the card may be requested by the cardholder; however, any other additional benefit must be requested by the main cardholder, for which the cardholder must present its valid card and identification. The accumulated doses in each card are not transferable to third parties and may not be used after their expiration date.
7.3. Hospital Clínica Bíblica reserves the right to include the new card or to combine said card with other credit or debit cards linked to the Preferred Customer Program.
8. Dose Accumulation and Discounts
8.1. One dose is earned for every dollar ($1) purchased in outpatient services as well as in admissions at Hospital Clínica Bíblica; except Pharmacy where one dose will be granted for every two dollars spent ($2).
8.2. From the moment members present their Preferred Customer card for the first time, and every subsequent time while using any service at Hospital Clínica Bíblica, the cardholder will accumulate one dose in his or her personal account. Two doses per dollar ($1) spent in outpatient or inpatient services, with the exception of Pharmacy where one dose will be granted for every two dollars spent ($2).
8.3. The Classic card will apply only for the accumulation of doses and a 5% discount in Laboratory services, Image Diagnostics, Pharmacy and Emergencies.
8.4. The Gold car will grant doses and a 5% discount in Laboratory services, Image Diagnostics, Pharmacy and Emergencies as well as inpatient services in the Hospital.
8.5. The Platinum card will grant doses, 5% discount in Laboratory services, Image Diagnostics, Pharmacy purchases and Emergencies, inpatient services in the Hospital as well as a Basic Medical Checkup, which must be used during the calendar month of the customer's birthday, if the customer has spent at least one thousand dollars during the past twelve months. These benefits will apply to those customers that already hold a Platinum Preferred card as it is no longer being granted since October first two thousand eleven because of re-launching of the program.
8.6. Hospital Clínica Bíblica reserves the right to implement projects and promotions for all or part of the coder, that grant additional benefits aside from the ones stated in this regulation.
8.7. For promotions where discounts for specific services or seasonal services, packages, insurance, business agreements or promotions, the discount offered by the Preferred Customer card will not be applied, regardless of its category. Likewise, other benefits for medical fees described herein will not apply.
8.8. The accumulation of doses is applicable to all types of insurance, agreements or any other promotion by ASEMECO.
8.9. ASEMECO will use the currency exchange rate for dollars / colones published by the Central Bank of Costa Rica, HSBC bank and BAC San Jose bank, for invoicing purposes.
9. Dose Redeeming
9.1. Cardholders must accumulate at least one thousand (1,000) doses in a maximum term of six months to be able to redeem its equivalent in colones through the cash back discount system.
9.2. Every dollar spent on services at Hospital Clínica Bíblica equals one dose. For redeeming doses for hospital medical services, each dose equals two United States cents.
9.3. The doses, which the cardholder is entitled to on the categories here stated, may be applied at any moment for payments at the Hospital or ASEMECO subsidiaries. For such purposes, the cardholder must present his or her card and a valid identification for Costa Rican citizens or a
legally valid identification for foreigners. The accumulated doses are for personal use and are not transferable. Doses expire after six months for all cases and purposes. Cutoff dates for such purposes are on September thirtieth and March thirty-first of each year, dates after which they lose all validness and effect to be applied to a specific account.
9.4. The status of each member of the Preferred Customer Program, as well as promotions or other offers will be communicated by means of a newsletter. ASEMECO reserves the right to modify benefit conditions offered by the program, which must be understood and accepted by the beneficiaries.
9.5. The cardholder may use the Program card without limit, during the current year. In other words, there is no minimum or maximum of usage times that the card may or must be used, while valid.
10. Final Provisions
10.1.In case ASEMECO should withdraw the program before its validity expires, it is understood that ASEMECO will not be responsible for awards or benefits not redeemed to that date.
10.2. Rights stated on this contract may not be transferred, signed over or taxed or alienated in any way in favor of third parties.
11. Revocation
11.1 The present ruling revokes any other previous ruling and Hospital Clínica Bíblica is not responsible for changes affecting users of the previous program.
12. Transitory
These provisions take effect on October first two thousand eleven and shall remain in force until December thirty first of the year two thousand eleven. They may be extended at the sole discretion of ASEMECO until a new fidelity program from Hospital Clínica Bíblica is publicly presented with its corresponding Preferred Customer cards.
