Rules and regulations of the Preferred Client Program of the Clínica Hospital Bíblica


Cliente preferente1. General terms and conditions

1.1 The present Preferred Client Program belongs to the Association of Costa Rican Medical Services, hereafter referred to as ASEMECO, owner of the registered brand Hospital Clínica Bíblica”.

1.2 Clients of the Clínica Bíblica Hospital who meet the requirements defined in the present informatory document will participate in the present program. ASEMECO reserves the right to include other beneficiaries at its discretion.

1.3 Those clients who participate in this program will do so with the understanding that they accept and submit to the following conditions and limitations and to any modification that will take place in this regulation.

1.4. ASEMECO reserves the right to modify or cancel either partially or totally, whenever it considered necessary, its rules, norms, privileges, benefits y prizes.

1.5 It will be indispensable for the affiliated entity to adjust strictly to the conditions of special treatment that the program establishes for the members of the Preferred Client Program.

1.6. ASEMECO reserves the right to vary the benefits or special offers of the Preferred Client Program.

1.7. The Preferred Client Program is a personalized program. Correspondence and published material, related to the program, will be sent to the indicated address in the document of affiliation of each member, or else will be communicated by means of the telemarketing system, electronic mail, ordinary mail or any other means of communication acceptable to the beneficiary.

1.8 It will be the responsibility of the affiliated entity to notify the Clínica Bíblica Hospital about any change of domicile, post office box, electronic mail or telephone to guarantee the adequate flow of information between the parties.


Cliente preferente2. Definitions

2.1. ASEMECOis the name of the Association of Costa Rican Medical Services in the present regulation.

2.2 Program: Preferred Client Program; mechanism through which the people who use the services of the Clínica Bíblica HospitalHospital and comply with the requirements indicated herein can obtain additional benefits.

2.3 Affiliated member: Beneficiary of the Preferred Client Program, who has complied with the stated admission requirements, joined the program and enjoys the respective benefits.

2.4 Dose: Benefit for those affiliated members of the Preferred Client Program who use the ambulatory and hospitalization services of the Clínica Bíblica Hospital.

2.5 Points: Total of doses obtained by using the services of the Clínica Bíblica Hospital during a determined period.

2.6 Special offers: Activities of the Clínica Bíblica Hospitalduring a specific period of time with additional doses or other benefits.

2.7 Benefits: The total of the doses accumulated by the affiliated members for use in goods and services, with applied discounts, provided by ASEMECO through the Clínica Bíblica Hospital.


3. Period of validity

3.1 The validly of the present program is for a period of one year starting from 1 October 2013.

3.2 The information presented in this regulation will be valid as long as no new regulation replaces it.


4. Image rights

4.1 To use the benefits of the Preferred Client Program,the affiliated member authorizes the Clínica Bíblica Hospital to publish, whenever it is convenient, his name and photograph in publicity about the program. The affiliated member waives the fee for image rights in this case.


5. Concerning the doses and point modification

5.1. ASEMECO reserves the right to correct, eliminate or discount from the accounts of the affiliated members any purchases or doses that have been accredited erroneously.

5.2. ASEMECO reserves the right to award purchases or supplementary doses on the occasion of specific promotional campaigns.


Cliente preferente6. Membership

6.1 To form part of the Preferred Client Program, affiliation is made on-line by filling out the application form on the Web site www.clinicabiblica.com, with personal data. To receive the benefits it will be necessary to present one of the following identification documents: cedula, resident cedula or passport.

6.2 Such memberships will be for a period of one year and will be renewed automatically at the determination of ASEMECO.

6.3. ASEMECO has a total of eight business days to decide on the acceptance of the application.

6.4 Participation of any functionary of the following legal entities is forbidden from the Preferred Client Program: ASEMECO, ANVECO S.A., SITERE. Also, professionals and companies that provide services in or for any of the legal entities mentioned in the present clause are also excluded from participation in the present program, unless expressly authorized in writing by ASEMECO.

6.5. ASEMECO reserves the right to review the quantity of monthly purchases accumulated by its members to guarantee the levels that justify and extend the permanence of the members in the program in accordance with the regulation.

6.6 The permanence of a member in the program can be suspended until there is a satisfactory solution to any discrepancy or detected anomaly.

6.7. ASEMECO reserves the right to terminate the membership of any affiliated member who, in the judgment of ASEMECO, has committed fraud, abuse or violation of any of the natural processes of purchase, use of benefits, use of awards, or any other point of the regulation of the program. To be disqualified, an affiliated member will lose all the membership rights. All of the above without prejudice or waiver on the part of ASEMECOof judicial and extrajudicial actions can be initiated, depending of the damage caused.

6.8 In case the affiliated member resigns from the program, he will automatically lose the rights on his purchases or accumulated doses, which may not be transferred to another member. Resignation takes effect once the affiliated member completes the application for disenrollment in writing or when during a three month period the affiliated member has not made use of his affiliation in the Preferred Customer Program membership for more than three months.


7. Affiliation

7.1 Every new member of the program who has become affiliated through the Web site www.clinicabiblica.com will find information on-line which identifies him as a Preferred Client. 7.2 The doses or accumulated points for each affiliated member are not transferable to third parties nor can they be used beyond their expiration date.

7.2. The doses or points accrued by each member are not transferable to third parties and cannot be used after the expiration date thereof.

7.3 The Clínica Bíblica Hospital (ASEMECO) reserves the right to include a new beneficiary or to combine this program with others related to the Preferred Client Program.


Cliente preferente8. Accumulation of doses

8.1 One dose is accumulated for each dollar ($1) of purchase in both ambulatory services and hospitalization at the Clínica Bíblica Hospital, except in the case of the pharmacy in which each purchase is worth two dollars ($2).

8.2 From the moment of affiliation for the first time in the Preferred Client Program and every time that he makes use of the services at the Clínica Bíblica Hospital, the affiliated member will accumulate doses for his personal account. These doses for ambulatory services and hospitalization will be accumulated at the rate of one dose per dollar ($1) spent. The exception is the pharmacy in which purchases will be assigned one dose per every two dollars ($2) spent.

8.3 The Clínica Bíblica Hospital reserves the right to implement projects and special offers for all or some of the affiliated members to allow them to obtain additional benefits to those in the present regulation.

8.4 These benefits are not applicable in the payment of medical fees.

8.5 The accumulation of doses is applicable in all types of insurances, agreements or other special offers made by ASEMECO.

8.6 To calculate the value of the dollar with respect to the colon in billing, ASEMECO will use the average of the exchange rate of the Banco de Costa Rica, Banco Davivienda, and Banco BAC San José.


9. Redemption of doses

9.1 It is necessary to accumulate a minimum total of one thousand (1 000) doses in a maximum period of six months to be able to apply its equivalent in colones through the system of reimbursement through discounts.

9.2 Every dollar used to purchase services in the Clínica Bíblica Hospital is equivalent to one dose. For the redemption of doses for medical services of hospitalization each dose is equivalent to two cents on the dollar.

9.3 The doses of the affiliated member may be applied at any moment of payment to be made at the hospital or any of the subsidiaries of ASEMECO. In this case, the client must present identification of his accreditation and the corresponding cedula for Costa Rican citizens or the legal representative in the case of foreigners. The accumulated doses are of personal use, are not transferable and the right to use them expires in all cases every six months. The cut-off dates every year are 30 September and 1 March, at which time the doses lose all validity and effect to be applied to a determined account.

9.4 The account statement of each affiliated member of the Preferred Client Program, special offers or other deals will be communicated in an informative bulletin. ASEMECO reserves the right to modify the conditions of the benefits offered in the program, which must be made known and accepted by the beneficiaries.

9.5 The affiliated member may use the program of affiliation for an unlimited time during its period of validity.


10. Concluding provisions

10.1 Should ASEMECOwithdraw from the program before the expiration of its period of validity, it is understood that ASEMECO will have no responsibility whatsoever for the awards or benefits which at the time of withdrawal from the program have not been redeemed.

10.2 It is not possible to transfer, cede, tax or sell in any fashion the rights stipulated in this contract in favor of third parties.


11. Repeal

The present regulation repeals any previous regulation and the Clínica Bíblica Hospital is not responsible for changes to the users of the earlier program.

Allow us to inform you that from 1 October 2013, the Preferred Client Program will be making the following adjustments:

A. Card use

To identify and receive the benefits of the Preferred Client Program, the presentation of the plastic card will not be necessary. These benefits can be obtained with the presentation of the cedula of identification.

B. Discounts

With optimism allow me to inform you that discounts will be substituted by special offers and other benefits which will be communicated to you.

C. Accumulation of doses and points

You may continue to accumulate doses or points based on the volume of purchases you make and on your participation in special offers and other benefits. The expiration of the doses and points will be six months from the time of purchase.

Assuming that you as a cardholder has complied with the following point of the regulation of the Preferred Client Program published on the Web site www.clinicabiblica.com:

  • "1.8 The cardholder will be responsible for notifying the Clínica Bíblica Hospital about any change in domicile, post office box, electronic mail or telephone to guarantee an adequate flow of information between the parties.” Allow us to reiterate that these stipulations are effective from 1 October 2013. Evaluación Deportiva Pre-Temporada


Dirigida a deportistas Amateur y Elite.

- El mayor problema que presenta actualmente en deportistas, es que durante la preparación a su evento deportivo se dan recargas y lesiones y deben parar su entrenamiento.

- Zonas problema o vulnerables por lesiones anteriores, se valorarán para mejora de “performance” deportivo acompañando al atleta durante su preparación al evento que participará. Ej: Maratón de Chicago, IronMan, etc.

- Enfoque correcto de como debe fortalecer y rehabilitar la zona muscular y/o articular para protección del atleta durante su deporte.
