Patient records subsidiary

This is a subsidiary of the Medical Management Department (Dirección Médica), which specializes in the management of the information system of health services of the Clínica Bíblica Hospital. Among our responsibilities are the following:

• Custody of the clinical information of our patients (clinical health record)
• Elaboration of health statistics
• Supervision of the quality of medical records
• Making available clinical health records to guarantee the continuity of attention and evolution of our patients

Committee of clinical health records

This is a technical entity charged with organizing, according to national and institutional norms, the regulations about procedures, use, custody, conservation and conformation of clinical health records.

It is made up of the following services: Medical Management Department, Nursing Department, Legal Services, Medical Records, Admissions, and Hospital Unit of Quality Care.

Confidentiality

A clinical health record is made for each patient who receives attention in our institution to guarantee the integration and continuity of attention. The health record is a legal medical document, which at all times is monitored to control the quality of the clinical information and protection of confidentiality given to users.

The information contained in the clinical health record is confidential, as required by the regulations in force in the country and we ensure the fulfillment of this confidentiality with the application of protocols, regulations and policies in the Hospital.

For this reason at the Clínica Bíblica Hospital and according to Articles 189,190 and 191 of the General Regulations for Hospitals of the Republic of Costa Rica, it is strictly forbidden to facilitate clinical health records to third parties, except with written authorization of the patient, duly authenticated and complying with the requirements of the stated law.

Certificates of birth, death, epicrisis, and others

Epicrisis

An epicrisis is a clinical summary of a hospitalization or an outpatient visit of the patient. This summary might include:

• Reason for hospitalization or consultation
• Diagnosis for admission or discharge
• If a surgical procedure took place
• Medical treatments
• Condition of hospital discharge

To process an epicrisis, a person can do the following:

• Call our central telephone exchange directly at 2522-1000 to request the process.
• Make your request by electronic mail at contraloriadeservicioalcliente@clinicabiblica.com. It is necessary to include a copy of the patient´s identification document).
• If you come personally, please bring the patient´s document of identification and go to the reception area in the south lobby of the Clínica Bíblica Hospital.
• Complete the form requesting medical documents (Formulario de Solicitud de Documentos Médicos: FO.RM-012), specifying the date of the medical consultation and hospitalization.
• The process takes 8 business days and you will be notified by telephone when it can be collected.

In case the certificate is requested or collected by a third party, he must provide documents of identification and authorization in writing by the patient, duly authenticated by a lawyer and in compliance with the regulations established in the General Regulations for Hospitals of the Republic of Costa Ricain Articles 189, 190 and 191.

Birth Certificate

The birth certificate is an official document of the birth of an individual, which is signed by the interested parties in front of a Registry assistant of the Civil Registry. To request a birth certificate:

• The father or mother must be present to sign a birth certificate within eight days after the birth.
• Either parent must bring an identification document (cedula, passport or resident cedula)
• Verify that all the vital data are correct.
• The father or mother shall verify that all data specified in the vital statistics form are correct.
• If the mother is single, married (when the birth takes place within the 365 days after the civil ceremony), in a domestic partnership, legally separated or divorced, and the baby is born after the 300 days of judicial separation or divorce, both parents must be present to sign the birth certificate.
• In cases in which the parents were underage at the time of the birth, they must be accompanied by an adult with a degree of kinship.
• If the parents are foreigners or were married abroad, the father and mother must be present to register the birth certificate at the respective office of the Hospital.
• When the birth occurs after the 365 day period following the wedding, one of the parents must sign the birth certificate.
• Each week the Department of Medical Records sends the certificates that have been signed by the parents to the Civil Registry for due process of inscription.

To request a Certification of Birth, the person must:

• Present himself personally at the reception area of Medical Records with his identification document.
• Complete the form requesting medical documents (Formulario de Solicitud de Documentos Médicos: FO.RM-012), specifying the date of birth and the complete name of the mother.

*The process takes 8 business days and you will be notified by telephone when the document can be collected. If it is solicited by a third party, it will be necessary to present identification documents and authorization in writing, duly authenticated by an attorney, and to comply with the requirements established by law in the General Regulations for Hospitals of the Republic of Costa Ricain Articles 189, 190 and 191.

The certification of death is an official document that certifies the death of an individual. Certifications are solicited for different purposes, by different institutions like: embassies, banking institutions, life insurance companies, attorneys to process legal procedures and special requests of the Civil Registry.

To request a certification of death the person must:

• Present himself personally with his identification document. (The interested party must have some degree of kinship with the deceased).
• Fill out a form in writing.
• Present a certification from the Civil Registry stating his kinship (Certification of Birth or Certification of Marriage) at the reception of Medical Records.
• • Complete the form requesting medical documenta (Formulario de Solicitud de Documentos Médicos: FO.RM-012).

*The process takes 8 business days and you will be notified by telephone when the document can be collected.

Certification of Declaration of Death

This is the official document that declares the death of an individual at the Clínica Bíblica Hospital and is signed by the interested party, the treating physician and the Registry assistant of the Civil Registry.

To receive the Certification of the Declaration of Death the person must:

• Present himself personally with his identification document. (The interested party must have some degree of kinship with the deceased).
• Verify that all the vital data in the Certification of the Declaration of Death are correct.
• If the data on the Certification of the Declaration of Death are correct, the interested party must sign the document which he receives.
• Each week the Department of Medical Records sends the certificates to the Civil Registry for due process of inscription.
