Letter of invitation to the program

Esteemed Colleague,

Everyone knows of the importance of an accurate diagnosis, a timely treatment and an adequate and comprehensive follow-up for the patients with a diagnosis of heart failure or cardiac insufficiency (CI), since they have consistently shown the reduction of major cardiovascular events.

The Heart Failure Program was established at the Clinica Biblica Hospital more than a year ago, with the most important elements to ensure the objective of reducing hospitalizations and mortality related with heart failure.

In the first place, we set up a multidisciplinary team of specialists in the most sensitive areas such as Intensive Care, ER, Cardiac Rehabilitation, Geriatrics, Cardiovascular Surgery, Electrophysiology, Interventionism and Non-Invasive Diagnosis. Then, and as part of the backbone of the HFP, the Nursing Team was also set up with three trained and experienced professionals.

In addition, we have a program of specific digital records integrated with the electronic file system of the Hospital, and also applicable for out-patients, thus enabling the assessment of the different clinical variables, of diagnosis, therapeutic, including the international classification, progress reports and the nursing assessment and follow-up tools.

All of the above supports a full protocol that goes from the inclusion criteria to the personalized follow-up, adjusted to the most accepted international guidelines, without additional cost and without dispensing with your family doctor, where you always remain the leader and we are the permanent support for the patient and his/her caregivers.

Nursing personalized follow-up, with the education of the patient and/or caregivers through printed educational material, delivery of digital sphygmomanometers and scales with the adequate training for their use.

Periodic reports after each contact or special situations.
Submission of the most complex cases to the multidisciplinary team.
Registration of all the information into a specific database for HF.

Inclusion Criteria

Patients with confirmed diagnosis of heart failure with a reduction of the systolic function with an ejection fraction of the left ventricle equal to or less than 40%.
That they have a caregiver who can be a relative or someone who accompanies them.
Availability to receive education and follow-up from the nurses and comply with the recommendations offered, both for the patient as well as the caregiver.

How to reference a patient?

Comply with the inclusion criteria established.
Contact the HFP staff for the notification that the patient is being sent there.
